• Cocky anesthesia
• Wrong sized tube
• Unstocked cart
• Allergies not communicated

Examples:

• On induction, patient becomes difficult mask when pre-oxygenating. Desat, redness/blanching/hives. (epi, steroids, assist anesthesia, anticipate difficult intubation vs trach)
