Outline
Nursing Points
General
- Important points for testing
- Massive blood transfusion–> Large amount over 24-hour period
- High amounts of citrate which can cause hypocalcemia
- Citrate = Preservative in blood donation bag
- Binds to calcium and inactivates it
- Citrate then reduces calcium level in blood
- Removed from blood by liver quickly but not in large amounts and not in trauma patients
- TPN (Total Parenteral Nutrition)
- Creats increase in insulin
- Causes cellular shift of phosphate
- Results in hypophosphatemia
- Causes cellular shift of phosphate
- Creats increase in insulin
- Amphogel-> Medication for gastric acid and it reduces phosphate levels
- ARDS ( Acute Respiratory Distress Syndrome)
- Fluids leak into lungs–>Causes difficulty breathing
- Decrease fluids in these patients
- Acidosis–> Increases potassium
- MCL1 (Modified Chest Lead, Marriott’s Chest Lead, Modified Central Lead)
- On 12 lead EKG
- Negative electrode below left clavicle
- Positive electrode on 4th intercostal space
- MCL1 will differentiates Ectopy vs. Aberrancy
- On 12 lead EKG
- Massive blood transfusion–> Large amount over 24-hour period
