- Intervene as needed
- Percutaneous Coronary Intervention (
PCI) - Percutaneous Transluminal Coronary Angioplasty (PTCA)
- Stents (with or without drugs) -> Educate patient on continuing aspirin and Plavix
- Extraction Atherectomy
- Percutaneous Coronary Intervention (
- Assess pressure in left ventricle
- View mitral and aortic valves
- View coronary arteries
- Prevent nephropathy induced by contrast
- Low dose
- Hydrate with NS
- Mucomyst day before (vasodilating)
- Hold drugs that affect kidneys (24 hours before procedure)
- Some antibiotics
- Cyclosporine
- NSAIDS
- Metformin
- Post-procedure
- Pressure 20-30 minutes
- Vitals
- Assess legs (pulse checks, temperature, color, pain)
- Assess site for bleeding internal and external
- How often? -> q 15 min. for one hour, q 30 min. for one hour, then hourly
- Bedrest and leg straight for 2-4 hours after procedure
- Complications
- Hematoma
- Thrombosis
- Retroperitoneal bleed
- MI
- CVA
- Death
- Renal dysfunction
- Acute Coronary Syndrome
- Heart isn’t being perfused with oxygen
- Treat immediately
- 20 minutes causes irreversible injury
- Unstable angina
- Non-stemi
- Stemi (candidates for reperfusion therapy)
