Patient Robert Hernandez is a 65 year old male received to the cardiac surgical floor from post-anesthesia care following a scheduled right lung wedge resection.

1500: Patient Robert Hernandez is a 65 year old male received to the cardiac surgical floor from post-anesthesia care following a scheduled right lung wedge resection. Patient has a history of newly diagnosed lung cancer, hypertension, and GERD. Upon arrival to the floor, patient is alert and oriented and rates pain 5/10 to the right rib area. Surgical incision is covered with a clean gauze dressing. Chest tube to right side connected to suctions per orders with sanguineous output. Breath sounds are diminished in bilateral bases, clear in bilateral upper lobes.

• Chest tube to -20cm water suction
• Morphine 2mg IV q2h PRN pain > 4/10
• Heparin 5000 units SQ q8h
• Docusate sodium 100mg BID
• Clear liquid diet, advance as tolerated
• Up to chair in AM, ambulate twice a day
• Incentive spirometer 10x per hour while awake
