As part of the hospital’s quality improvement plan, the Code Blue team is reviewing the chart of a recent in-hospital cardiac arrest. The goal of the review is to identify opportunities to improve care. The team is looking to identify if the aspects of high-quality CPR were performed during the resuscitation.

Upon entering the client’s room, the client was found unresponsive, with no breathing and no pulse. The hospital Code Blue Team was immediately activated. Nurse remained at bedside awaiting the arrival of the Code Blue Team. Upon arrival to the client’s room, the Code Blue Team leader instructed team members to start chest compressions using a 30:2 compression to ventilation ratio. Each breath was given over 3 seconds. Chest compressions were noted to be at least 2 inches with full chest recoil after each compression. CPR was paused for approximately 30 seconds to allow for placement of an advanced airway. Family members remained in room with primary nurse. Team members switched compressors every 5 minutes. After 10 minutes, it was noted that there was a return of spontaneous circulation and the client was transferred to the intensive care unit. The rate of compressions was maintained at 100-120/minute during the resuscitation.
