The 58 year old male client arrives to the Emergency Department (ED) complaining of shortness of breath and chest pain. He seems restless. Over the past few days the client states that he has had right calf pain and difficulty walking. He says his breathing is short and he does not feel that he can get enough air. The nurse has reviewed the client’s history and physical and has documented his reported information regarding the purpose of his visit. The nurse is now determining relevant information that will be important in addressing his current acute needs.

I feel like I am not getting enough air into my lungs.” Upon removing his shoes and socks the nurse notes that his right calf is swollen, red, and hard. The right calf is substantially larger than the left.  The client states that he has been urinating normally.
